  Welcome to Burbage Surgery...

Requests should be handed in or posted to the surgery. Please allow 48 hours notice before collecting your prescription. Alternatively, if you supply a stamped addressed envelope, we will forward your prescription to you. It is also possible to request your repeat medication on line once you have registered with the practice for on line access.

Please mark clearly on your request the items that you require. Every 6 to 12 months you will be asked to have your medication reviewed to ensure that you still require each drug. It is a requirement of the practice that every patient must have his or her medication reviewed annually. Failure to attend for medication review may result in the practice refusing to issue further repeat medication until you have seen the Doctor.

You may find that the name of your medication has been altered on your prescription and your repeat slip. If this happens it is most probably that the doctor has changed your medication from the brand name to the chemical (generic) name for that particular drug in accordance with practice policy. There should be no difference in the drug itself and you should not notice any difference. However, should you wish to discuss this with your doctor, please make an appointment in the usual way. Please only request medication that you require. If you do not require an item please do not order it. Sometimes, your Doctor or the hospital may alter your repeat medication. It is important, therefore, that patients should keep no more than one month's supply of medication at any time. All medicines carry expiry dates. If you find that the date on your medication has expired, please dispose of this safely.

NHS Direct is a 24 hour telephone helpline. The helpline is staffed by nurses who can help reassure you and give you telephone advice and health information. Anyone can ring NHS Direct at any time for health advice.

If you feel that there is something wrong with you or someone you are called about, a NHS Direct nurse will be able to help you and:

- tell you whether the symptoms can be managed safely at home and advise you on what to do to treat yourself or the person you are worried about.

- advise you if you do need to seek further help from a medical professional and direct you to the right service. In an emergency the nurse can also transfer you directly to the ambulance service.

NHS Direct is staffed by experienced nurses who are especially trained to give advice over the phone. They will ask you a series of questions which will help them decide how serious your problem is.
